1. Each NASCAR member with a competitor or pit permit, and who has signed the release sheet for the meet for which the competitor or pit permit is issued, is entitled to benefits as arranged by NASCAR if he is accidentally injured as a result of external violent and visible means while participating in said meet. All competitors in NASCAR sanctioned events agree to abide by the decisions of the Benefit Plan officials in administering the Benefit Plan. The Benefit Plan applies only at NASCAR sanctioned events and not while en route to or from a racemeet [sic]. 2. No member will be allowed in the pit area until he or she has secured a competitor or pit permit -- and signed the release sheet for that particular racemeet [sic]. 3. Competitor or pit permits are not transferable and are not to be signed by anyone except the person to whom issued. 4. Any member returning to competition after injury shall be deemed to be physically fit and further disability benefits and/or medical benefits shall cease as of that date. Any member returning to his usual occupation after injury shall not be entitled to further disability benefits as of that date. 5. Any member involved in an accident while on the racing premises and who does not report to the NASCAR official in charge of the event before leaving the premises (providing such member is able to make such a report) will not be eligible for benefits prescribed under the Benefit Plan. 6. The 1974 Benefit Plan schedule is as follows: a. Total disablement, $1,560.00 -- This will be paid at the rate of $60.00 each week, starting from the eighth day, but not exceeding altogether 26 consecutive weeks for any single disablement which prevents the NASCAR member from engaging in his usual occupation or any racing events. (Disablement must begin within 30 days of accident). b. Blanket medical, $15,000.00 -- Including expense of treatment by a physician, surgeon, chiropractor, osteopath, nurses, hospital and other professional services rendered and payment may be made directly to these people for the account of the injured NASCAR member. (Treatment must commence within 26 weeks of accident). c. Death--$15,000.00 will be paid to the named beneficiary, less reasonable funeral and burial expenses which the Benefit Plan of the Competitor Liaison Bureau of NASCAR, Inc., is authorized to pay. d. Dismemberment or loss of both arms, legs or sight of eyes, $15,000.00. Loss of one arm, leg or sight of eye $7,500.00. (Maximum $15,000.00.) (Loss under sub-paragraphs c or d must occur within 9 months of date of accident, and payment of benefits under either item will preclude payment under the other.)
